Hike Registration

To sign-up for a hike, fill-in the information below, read the notes and agree to the Recognition of Risk and
Release of Liability paragraph, then press the submit button at the end of the form to send it to us.

You will be contacted at the email address you provide regarding meeting places, times and other arrangements
(including carpool sharing costs)... and to answer any questions you might have.
All members of a group on a single registration will be registered for all hikes chosen on that registration.  So if all
individuals in your group do not want to go on the same hike(s), a separate registration will be required for each.

Note: These hikes are planned for visitors to the Eugene area during the 2008 Olympic Trials and
The Oregon Bach Festival.  Obsidian members should not register for these hikes.

PLEASE NOTE

•  The leader may refuse to allow any person to participate on this trip, for any reason. The leader’s decision is final.
•  You are responsible for clothing, equipment and all other supplies to keep you safe and comfortable during the trip.
•  No dogs, firearms, illegal drugs or alcohol are allowed on any Obsidian trip.
•  Be prepared to contribute your share for car pooling expenses. The amount suggested per person is $.07 per mile.

 

Recognition of Risk and Release of Liability

By selecting Accepted in the drop-down box below,  I am acknowledging that I am aware that outdoor activities can be dangerous and present occasional hazards not encountered in everyday life. I am prepared to deal with and accept all risks of this trip and associated activities as my own responsibility. I am responsible for providing my own equipment, including first aid supplies and medicines.
In consideration of OBSIDIANS, INC. sponsoring and granting permission for me to participate in this trip, I release OBSIDIANS, INC., its officers, agents and members from all claims on account of my death or injury resulting from any act, or failure to act, even if negligent, during my participation in this trip, including travel to and from the trailhead, and any associated activities.
Trip leaders will require all adult trip participants to also sign hard copy registration/release forms for themselves and their minor children before embarking on their trips.
